It's an ambulance. In Hyakugojyuuichi, the Emergency Pee Wees traveled in it on their way to save Jay Jay after the latter was shot by the Machine Gunner. Like many of the characters and props used in Hyakugojyuuichi, it has appeared in several other animutations since then.
